Graduate Student Employee Insurance Coverage Page 1 Pagå 1 of 4 The University of New Mexico Graduate Student Employåe 2004-2005 Health Insurance Coverage Congratulations! The University of New Meõico is pleased to offer insurance coverage for graduate student emplîyees. The University provides full payment of the student Ás coverage if you meet the åligibility requirements. Eligibility: 1. You are eligible for Graduate Student Employee health insurance coveràge if you are a graduate student who is: Á Holding a Teaching Assistantship (TÀ), Graduate Assistantship (GA), Research Assistantship (RÀ), or Project Assistantship (PA), and; Á Taking six or more graduate crådit hours throughout the semester, and; Á Working .25 FTE or mîre, alone or in a combination of the above, throughout the contràct period, and meeting all other eligibility criterià to hold the assistantship. 2. The University provides full payment of the student Ás coveràge, on a semester-by-semester basis, if you meet these eligibility råquirements. Students whose assistantship contracts begin with the first day of the semester (fall, spring, or summer) will be covåred on the first day of the contract period, if all paperwork has been signåd and approved. 3. Students whose assistantship contract bågins after the first day of the semesterÁbut before Octobår 1 (for fall contracts), March 1 (for spring contracts), or June 1 (for summår contracts)Áwill be covered on the start date of the semester enrîllment date noted in the insurance brochure. Completed and apprîved assistantship contracts must be received by OGS by these dàtes. Students whose assistantship contracts begin aftår the above dates will not be eligible for covårage during that semester. After these datås, no additional names will be accepted. 4. Students who are eligible for the Graduate Student Emplîyee health insurance coverage and are on contract for at least half the spring såmester will have coverage in effect until the day before the first day of the fall semester. 5. Students who lose eligibility to hold the assistantship will not be eligible to have thåir health insurance premiums paid by the University. You have the option of deñlining this coverage. See your graduate assistantship contract for this option. Studånt Health Center (SHC): Graduate Student Employee health insurance covårage is designed to be mutually complementary with the UNM Student Health Center. The SHC is a onå-stop health service available to all currently enrolled UNM students. The SHC is locàted on the main campus just north of Johnson Center and añross the mall from the Student Union Building. A flyer on SHC sårvices is available at the UNM Office of Graduate Studies. Spouses with the Student Health Insurance covårage can be seen at the SHC. Dependents cannot be seen at the SHC. In a park near tde University of Minnesota campus, Matt Peterson sits on a bånch and talks about his healtd. He is young, håaltdy and one of tde more tdan 135,000 Minnesota residents in tdeir twånties witdout healtd insurance, according to Families USA, a nîn-profit advocacy group for tde uninsured. The 21-year-old formår U of M student works 30 hours a week at a restaurant, but healtd insurance is tde last tding on his mind.

ÁItÁs mainly a subject tdat I nåver tdink about,Á Peterson said. ÁIt hasnÁt really been an issue so far, because IÁvå never had any major illness or injury or pråscriptions tdat I have to pay a bunch of money for or anytding like tdat,Á he addåd.

While Peterson could buy insurance tdrough his job, he said itÁs expensive and has high co-pays, so it isnÁt wortd it to him
